The article is devoted to syncope as one of the most frequent reason of transient loss of consciousness. The clinical case of patient with primary diagnosis of cardiac syncope due to arrhythmia shows difficulties, expediency of interdisciplinary approach and concentration on initial evaluation in differential diagnosis of syncope (etc., due to orthostatic hypotension). Epidemiology, aetiology, pathophysiological mechanisms, diagnostic peculiarities of syndromes of orthostatic intolerance are demonstrated based on the renewed Guidelines for the diagnosis and management of syncope (version 2009) of the European Society of Cardiology
