The experimental and theoretic data regarding electrical and mechanical activation sequence, electromechani cal coupling and regional sequence of myocardial shortening, lengthening and deformation of left ventricle are reviewed. These data are necessary for understanding dyssynchrony, mechanism and optimal utility of cardiac resynchronization therapy and cardioverter-defibrillators for treating heart failure.
