The current concepts regarding microvolt T-wave alternation, methods of its quantitative evaluation are presented. Recent clinical trials evaluating the prognostic utility of T-wave alternans in patients with coronary artery disease, depressed and intact left ventricular ejection fraction, ischemic and non-ischemic cardiomyopathy etc. are summarized. The findings of these studies are discussed in the context of implantable cardioverter-defibrillator usage.
