Left ventricular (LV) systolic dysfunction is a frequent and serious complication of acute myocardial infarction (AMI) which lead to increased risk of sudden cardiac death and heart failure. The article presents data of the analysis of clinical-anamnestic characteristics and instrumental observations in 448 patients with ST-elevation AMI. Prognostic factors and causes of the acute heart failure (AHF) development in AMI patients with systolic dysfunction LV are analyzed, as well as efforts at its prevention. Early diagnostics of the AHF manifestations and early aggressive treatment of this category of patients improve the short-term and long-term prognosis.
